JABA Program to Help with Transition from Hospital to Home

A new program is offering support for central Virginia seniors and adults with disabilities as they head home from a hospital stay.

The Jefferson Area Board for Aging (JABA) is accepting patients for its "Care Transitions" program. A Care Transitions coach will help plan their hospital discharge, do an in-home visit, and then follow up for a month.

“We try to see where we can connect them with community resources that might be able to help them and go through the various pieces they might be struggling with - whether it's medications or getting to doctor's appointments or finances - really the gamut,” said Mary Ellen Hasenfus, JABA Care Transitions coordinator.

The goal of the program is to prevent a patient from having to go back to the hospital.
